Job Summary The Patient Access Data Entry role will support a temporary Electronic Health Record (EHR) conversion project for the PACE program. This role is responsible for reviewing, validating, and entering patient information to ensure the accurate transfer of medical records and clinical documentation between healthcare systems. Essential Job Functions Duties include, but are not limited to:

  • Review patient charts, medical records, and clinical documentation for accuracy and completeness.
  • Enter, update, and maintain patient information within the electronic health record

(EHR) system.

  • Assist with medical record migration and data validation activities during system conversion projects.
  • Identify and report missing, incomplete, or duplicate information.
  • Navigate electronic medical records to review patient encounters, referrals, orders, and other clinical documentation.
  • Maintain patient confidentiality and comply with HIPAA requirements.
  • Collaborate with clinical and operational teams to support project goals and timelines.
  • Assists with document indexing; uploading, labeling, and categorizing scanned clinical documents to ensure accurate placement within patient charts and easy retrieval by clinical staff.
  • Process high volumes of patient charts to reduce documentation delays and support timely clinical workflows.
  • Verify patient information for accuracy and completeness and notify supervisor of correcting discrepancies to maintain data integrity.
  • Perform other project-related duties as assigned.
