Hybrid RN Case Manager | No Bedside | 2 Remote Days
About the position Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Registered Nurse (RN) Case Manager in Lynn, Massachusetts. This hybrid role focuses on coordinating care for aging adults living independently at home. It is a community-based nursing opportunity centered around care planning, continuity of care, and service coordination, with no bedside nursing responsibilities required. The position offers a hybrid structure combining community patient visits with remote follow-up and care coordination work within a mission-driven senior care environment with strong interdisciplinary collaboration.
Responsibilities
- Develop individualized patient care plans
- Coordinate community-based services and ongoing care needs
- Conduct patient visits within community and home settings
- Manage follow-up and care coordination remotely
- Collaborate closely with interdisciplinary care teams
- Support aging adults in maintaining safe independent living Requirements
- Active Massachusetts RN license REQUIRED
- Minimum 2 years RN experience REQUIRED
- Strong communication and organizational skills REQUIRED
- Comfortable working independently within community settings REQUIRED
- Team-based care approach REQUIRED
- Comfortable with hybrid remote/community workflow REQUIRED Nice-to-haves
